China is a large country with corneal blindness, which is characterized by high risk cases, especially chemical injuries, thermal burns and infections. According to statistics, there are 125 cases of eye chemical injuries per 100000 people in Hong Kong every year. There are no exact figures in the mainland of China, but due to the restrictions of work protection and conditions, the incidence of chemical and thermal burns is higher.
After studying the cases of ocular surface chemical burns in Shanghai from 1998 to 2008, some scholars found that most of the patients were young men, with an average age of (35.3 ± 14.8) years. It was difficult for traditional corneal transplantation to restore vision, and transplant rejection was the primary reason for the failure of penetrating keratoplasty. The failure rate of high-risk cases can reach 45%~65%, and the failure rate of chemical injury and retransplantation is higher.
For end-stage corneal blindness, especially severe dry eyes (such as Stevens Johnson syndrome and ocular pemphigoid at the end of the period) and eyelid atresia, corneal transplantation cannot be successful. In the past, the treatment of vision restoration was abandoned. However, artificial cornea has brought hope for such patients.
Artificial cornea is a high technology. The surgical techniques involving multiple sub specialties of ophthalmology are difficult to operate. The prevention and treatment of its related serious complications (such as corneal melting and leakage, displacement and excretion of artificial cornea, infection, etc.) is the key to maintain its long-term stability and visual function.
According to the current situation, the artificial cornea suitable for China's national conditions should at least meet the following conditions:
① No need for additional corneal grafts
② It has a wide range of indications, especially for the treatment of severe and end-stage corneal blindness
③ Safety during operation
④ The prevention and treatment measures for serious complications are easy to implement
⑤ High cost performance of artificial cornea
Throughout the domestic market of artificial cornea products, MIOK is the most in line with the above five requirements.
First of all, MIOK is made of artificial materials without donor cornea, which is helpful to alleviate the shortage of corneal donors in China.
Second, MIOK provides a new way for the clinical treatment of patients with contraindications of traditional corneal transplantation. It is applicable to patients with binocular corneal blindness who are difficult to succeed in corneal transplantation, including corneal transplantation failure, severe keratoconjunctival scar vascularization caused by chemical injury, thermal burn, explosive injury, eyelid atresia, severe autoimmune diseases (such as Stevens Johnson syndrome and cicatricial pemphigoid), and corneal blindness caused by dry eyes in the end stage.
Thirdly, the characteristics of MIOK are that the materials are hard medical materials: both titanium stent and PMMA materials have excellent biocompatibility, mechanical properties, low elasticity, corrosion resistance and strong processability. In order to improve the histocompatibility, the surfaces of the two materials can be modified to further improve the histocompatibility and the ability to heal and combine with surrounding tissues. At the same time, PMMA is an ideal optical material with excellent optical permeability (92%), and is widely used in ophthalmology, including artificial lens, artificial cornea and other implant optical products.
Fourthly, the central optical part of the Mikh artificial cornea is connected with the titanium stent through threads. The water tight combination between the lens column and the bracket with extremely high processing precision can prevent low intraocular pressure caused by water leakage and reduce the chance of intraocular infection. At the same time, Mihm artificial cornea is implanted by stages. Through intraocular perfusion and small corneal opening operation, serious intraoperative complications such as expulsion bleeding and leakage of Boston I and AlphaCor artificial corneas can be avoided to the greatest extent.
Most importantly, according to the clinical report, the common complications of Mihm keratoprosthesis are mainly the symptoms that are easy to deal with, such as residual cortex and tissue hyperplasia. Compared with other postoperative complications of keratoprosthesis, such as glaucoma, it is more convenient to deal with.
Fifth, the detachable design of MIOK can flexibly select the appropriate optical diopter and the height of the lens column according to the length of the patient's eye axis. Even after the operation, it can be disassembled and replaced according to the situation (the degree of replacement, the height of replacement and the worn lens column), without replacing the entire artificial cornea, which can save a lot of money.
To sum up, Mihm keratoprosthesis is more suitable for corneal blindness in China. You or your family and friends in front of the screen can call our official customer service number: 400-111-8801~
